### Why the tool list is not verified

app.omnara.com refuses an anonymous `tools/list`, which is the correct thing for a server holding real user data to do. Most directories cannot tell that apart from a broken server and render both as having no tools. It is not broken — it is gated, and it answered us to say so.

**Describe the bug**
A permission prompt will be sent but responding won't work.

**To Reproduce**
Steps to reproduce:
1. Do something Claude needs permission for.
2. Give it permission.
3. Observe the Claude instance, where the question is still active.
4. Observe the Omnara web UI, where the answer appears to send as text ("yes")

**Expected behavior**
Answering the question should pass back through to Claude correctly.
