Cache invalidation doesn't detect extraction code changes

Problem

The index cache (~/.cache/fedramp-docs/index-v1.json) only invalidates when the FedRAMP docs repo HEAD changes, not when our extraction code changes.

This means after updating extraction logic (like the recent case-sensitivity fix), users need to manually delete the cache:

rm ~/.cache/fedramp-docs/index-v1.json
