### Make skill tool calls auditable?

I need to be able to configure the `skills.rag` and `skills.analysis` tools such that I can pass a logging function at skill construction time (or via a reconfigure?):  if passed, each of the skill's tools uses that name to log the database path and the relevant params used to access the RAG db.

### fix: atomic rename race can blackhole documents via DELETE/UPSERT unique index conflict

## Problem

Editors like vim and `git checkout` perform atomic saves that fire `(deleted, added)` watchfiles events back-to-back. The `_handle_watch_change` code in `pollers/fs.py:113-131` checks `path.exists()` to suppress the DELETE when the file reappears immediately, but there's a timing window:

1. DELETE event fires, `path.exists()` returns `False` (file not yet rewritten), DELETE job enqueued
2. File reappears on disk
3. `Change.added` fires, tries to enqueue UPSERT for the same URI

### Feature: search_documents MCP tool should exhibit filter input

Hey, given that the Python API (`client.search(...)`) offers a `filter` argument, it would only make sense that the MCP tool `search_documents` also offers it. What do you think?

### FIle Not Found error with `docker run`

I'm following through the README.md in `/docker`, but am getting an error when running `docker run` as it's shown in the readme. I'm not sure if there's something missing on my end, or if there might be a step missing. Running with sudo, macos, Apple Silicon chip. Here's a dump of the terminal output:

### Read-only mode for the mcp server mode ?

Is there a way to launch the mcp server in such way the add_document_* and delete_document are not available ?
