### ncp code fails on Node.js 25 with 'Cannot read properties of undefined (reading length)'

## Environment

- **NCP Version:** 2.0.2
- **Node.js Version:** 25.2.1
- **OS:** macOS Darwin 25.2.0 (Apple Silicon)
- **Installation:** npm global (`npm install -g @portel/ncp`)

## Description

The `ncp code` tool fails with a JavaScript error when running on Node.js 25. The error occurs both via CLI and MCP interface.

## Steps to Reproduce

1. Install NCP on Node.js 25.2.1
2. Run `npm rebuild` in `/opt/homebrew/lib/node_modules/@portel/ncp`
3. Try to execute code:
