### adt_debug_listen never catches a debuggee: wrong Accept header on /debugger/listeners + missing stateful session

## Summary

`adt_debug_*` is currently unusable: `adt_debug_listen` **never** catches a debuggee, and even when a debuggee is attached manually, `adt_debug_stack` / `adt_debug_variables` fail with `noSessionAttached`.

Both problems are in the HTTP contract, not in the flow logic. The ADT debugger itself works fine over REST — I verified the full chain (breakpoint → listen → attach → stack → variables → continue) by hand against a real system, on both an ADT `classrun` request and a live Fiori/O
