importers: entity identity is wrong for schema-qualified DDL and inconsistent for indexes

Summary

Two entity-identity defects in the SQL importer. Both produce silently wrong or missing history rather than errors.

Schema-qualified DDL attributes the event to the schema

public.users is the default spelling in Postgres migration dumps.

packaging: three distribution surfaces have no test and have drifted

Summary

Packaging is solid where CI can see it — a real CPython 3.10.20 runs the full suite green, the [semantic] extra boundary holds under a genuine no-extra install, wheel contents are clean, and version sync across all eight surfaces is both correct and well-tested. selvedge supersede: missing --diff and --revisit-after (parity with the log command's supersede branch)

selvedge supersede is the documented, guided way to re-open a reverted decision, but it exposes only --reasoning/--constraint/--stale-when/--supersedes (plus attribution flags). It has no --diff and no --revisit-after.

The storage layer underneath (SelvedgeStorage.log_supersede) accepts and persists both fields, and v0.3.10 specifically fixed log_change dropping revisit_after/constraint/stale_when and diff on the supersede branch (#21) — so the raw path can record the re-app

log_change accepts revisit_after, constraint and stale_when on every change type, validates them, and then throws them away on two of the three branches. diff is dropped on supersede as well. No warning, no error — the call returns {"status": "logged", "warnings": []}.

### perf: the PreToolUse hook costs 34.6ms per gated tool call, 98% of it import cost

## Summary

The hook runs on every `Edit`/`Write`/`MultiEdit`/`NotebookEdit`/`Bash` call and spawns a fresh Python process each time. Measured p50 on the allow path — the ~99% case in a real session — is **34.6 ms**, of which the hook's own logic is **0.58 ms**.
