chore: optimize tool schema descriptions for token efficiency

Problem

Tool descriptions contain redundant words that cost tokens without helping the agent. Every token in a schema is paid on every conversation.

feat: discovery tool for dynamic module loading

Problem

Even with --modules, users must know which modules they need upfront. Agents can't discover capabilities at runtime.

Solution

Add an optional --discovery mode that registers a single meta-tool swiss_discover (~100 tokens at startup). The agent calls it to see available modules, then requests specific ones. Tools register dynamically via MCP's tools/list_changed notification.
