### Harden the contribute-fingerprint prompt: provenance preamble + timeout-as-signal fallback

`packages/elicitly/src/prompts.ts`'s `contribute-fingerprint` prompt is missing two hardenings that the hosted (Pro) counterpart gained after field-testing in Claude Desktop (elicitly-pro `d407068`). The free edition shipped its prompt in `elicitly` 0.5.0 without them:

1. **Provenance preamble.** Some hosts (Claude Desktop) deliver an MCP prompt to the model as a file-like attachment. A cautious model reads "You are helping the user…" arriving as a file as prompt injection and refuses. Fix: ope

### doctor() probe should bound its own elicitation timeout (below host tool-call limits)

`packages/tools/src/doctor.ts` calls `deps.elicit({ message, requestedSchema })` with no `timeoutSeconds`, so the probe inherits the MCP SDK's default 60s request timeout. On hosts whose tool-call timeout is also ~60s (Claude Desktop), the two race: the doctor is designed to catch the hung probe and finish the report with `advertised_but_unanswered`, but the client kills the call at the same instant, so the finished report never returns in-band.
