Dashboard: render markdown in conversation cards (agents can only send flat prose)

Problem

Agents can only send prose to the dashboard. The speak/announce tools document three markup features - **bold** (vocal emphasis + bold on the dashboard), inline tags like [pause], and wrapping tags like <soft>…</soft> - and nothing else. So when an agent has something inherently structured to report, it has two bad choices:

Claude's reply lands in the WRONG conversation when two sessions share a cwd or the user switches tabs

Repro (Krzysztof, 2026-07-20, personal Claude, two tabs)

Speak to thread 1 while viewing its tab, switch to tab 2 while thread 1 works — thread 1's spoken reply shows up in thread 2's history.

Root cause — the speak attribution chain guesses twice

  1. The MCP server identifies its agent via sessions.json[cwd] (server.py:_agent_name). The hooks of EVERY session overwrite that single per-cwd slot on every call — two sessions in one directory constantly steal each other's identity.

Zero-restart first install: MCP launcher should speak MCP itself and hot-add tools via tools/list_changed

Problem

On a FIRST install the plugin's MCP server is connected at session start, before the container exists. The stdio launcher (mcp_exec.sh) waits patiently, but the CLIENT gives up after its own ~30 s startup timeout and marks the server failed — waiting longer server-side is pointless. The user must reconnect (/mcp → reconnect) or restart once. #12 removed the reload for the container-restarts case; this removes it for first install.

Plugin install race: MCP tries to connect before the container is up, forcing a reload/restart

Problem (reproduced twice on clean plugin installs)

The documented first-install flow is: install the plugin, start the container, open the dashboard. But Claude Code connects the plugin's MCP server (http://127.0.0.1:8767/mcp) the moment the plugin loads — and at that point the container often isn't running yet. The connection is rejected, the server is marked failed, and it stays failed even after the container comes up. The user has to run /reload-plugins or quit and restart Claude Co
