[Bug]: Control Chrome encounters Unknown Issue(s)

After the installation of OpenWork completed, I have successfully connected it to OpenCode and run test queries on it. MCP servers from global config (~/.config/opencode/opencode.json) are ignored
Bug Description
The listMcp() function in packages/server/src/mcp.ts only reads MCP configurations from the workspace-level opencode.jsonc, completely ignoring global MCP servers configured in ~/.config/opencode/opencode.json.
This forces users to duplicate their global MCP configurations in every workspace, which is error-prone and inconvenient.

[Bug]: Local MCP servers are started twice on launch
Summary
On startup, every custom-configured local MCP server spawns two separate child process trees under the opencode sidecar. Only one is used; the second is an idle duplicate. Bundled MCP servers, such as imagegen-mcp-server, spawn only once, however.

- Configure local stdio MCP servers (either in opencode.json or via OpenWork Settings > Extensions).
- Launch OpenWork.

### Remote/sandbox provider OAuth callback fails to resolve
## Problem
Connecting a provider from a remote/sandbox environment does not complete because the OAuth callback target does not resolve.
## Current behavior
- Start provider connect flow in a remote/sandbox session
- Connect dialog shows: Failed to complete OAuth Request failed Request timed out.
- Browser opens a localhost callback URL
- Browser shows ERR_CONNECTION_REFUSED (localhost refused to connect)

### [Bug]: Desktop stays cloud-disconnected despite healthy Cloud MCP and fresh reinstall

OpenWork Desktop 0.18.39 on macOS Apple Silicon remains in an unstable Cloud/organization state even after a full clean reinstall. ### [bug] MCP Apps host does not apply opencode's {env:} config substitution, making correctly-configured MCP servers unreachable
## Summary
`mcp-app-host.js` reads the opencode config file directly and passes MCP `headers` through verbatim. opencode substitutes `{env:VAR}` before use; the Apps host does not. Any remote MCP server whose headers use the documented `{env:}` syntax therefore receives the literal template as a credential, and the Apps host can never connect to it.
Tool calls work (opencode makes those), so the server looks healthy. Only the interactive view fails.

### Rate limiting collapses to a single shared bucket behind a reverse proxy, and surfaces on `/mcp` as a misleading `invalid_mcp_token` 401
## Summary
On a self-hosted deployment behind a reverse proxy, Better Auth cannot resolve the client IP and
falls back to **one shared rate-limit bucket for the whole deployment**. Because the global limit is
20 requests / 60 s, a single active MCP client exhausts the bucket for everyone.
Worse, on the MCP transport the exhausted bucket is reported as:

### Dynamic client registration (RFC 7591) returns HTTP 500 with empty body — breaks native OAuth for all MCP clients
